Well-trained health workers can improve health-seeking behaviour
BONDO, 4 October 2010 (PlusNews) - Kenyan health workers have been
missing the opportunity to prevent HIV-positive people from infecting
others because they lack the skills and knowledge to counsel this
population, say specialists.

"Health workers have certain misconceptions about people living with
HIV... many don't have the training in how to provide messages to
those already infected," said Ann Okoth, the district public health
nurse in western Kenya's Bondo district. "So people who are positive
come [to health centres] and the opportunity to give information to
them is lost.

"Training and counselling for health workers dealing with HIV-positive
people is very important in improving people's health-seeking
behaviour and in turn for the success of prevention with positives
programmes."

According to a 2008 report by the Centre for Reproductive Rights and
the Federation of Women Lawyers of Kenya, negative healthcare
experiences can discourage HIV-positive women from seeking medical
care.

Among other violations, the report noted that women living with HIV
were frequently turned away from public health facilities or secluded
in an area of the hospital away from other patients; reprimanded for
bearing children or being sexually active, and denied access to
contraception, family planning and maternity services.

Training

National guidelines for promoting prevention among people living with
HIV were released in May and the government is now partnering with
JHPIEGO, an NGO affiliated to Johns Hopkins University, to train
health workers across the country to deliver messages about prevention
to their HIV-positive patients.

"For instance, when you test a mother who has come to the clinic for
antenatal care, the priority would be to have her bring her partner
for testing also and to help them in disclosure and to prevent mother-
to-child transmission of HIV," explained Tom Marwa, HIV/AIDS technical
officer for JHPIEGO.

Pregnant, HIV-positive and falling through the PMTCT cracks
So far 1,940 health workers - nurses, clinical officers, counsellors
and nutritionists - have been trained in counselling as well as how to
prevent unintended pregnancies, and how to screen for and treat
sexually transmitted infections.

A 2004 study showed that in Kenya, disclosing HIV status to partners
was associated with a four-fold increase in reported condom use, but
according to JHPIEGO, before the training, 69 percent of HIV service
providers in Kenya did not advise their HIV-positive clients to
disclose their status, while 32 percent did not discuss HIV prevention
with them. Afterwards, 80 percent of health facilities visited were
offering prevention with positives messages.

Couples' counselling

Leonida Ojuok is one of the patients at the Bondo District Hospital
who has benefited from the training. When she tested positive during a
routine antenatal visit, she was terrified of telling her husband.
Women are often divorced and thrown out of their marital homes after a
positive HIV diagno sis.

"I told the nurse that I was not going to tell my husband because he
would kill me... but she just told me to tell him to come for the next
visit," she told IRIN/PlusNews. "When we came together, she talked to
us and tested us.

"Then she told my husband - I was HIV-positive but my husband wasn't.
I am happy the nurse convinced him to support me... even today we have
come to the clinic together."

Identifying HIV-positive people

"The focus should not only be on those whose statuses are already
known and on ART [life-prolonging antiretroviral therapy] programmes
or some kind of support," Marwa said. "The 'healthy positives' are out
there spreading the virus without knowing it."

According to the results of a 2010 systematic review of behavioural
interventions targeting HIV-positive individuals in developing
countries, they were useful not only in providing HIV prevention
messages but in identifying HIV-positive individuals.

According to government statistics, an estimated 83 percent of HIV-
infected people do not know their status.

The authors of the review noted that because many HIV-positive
individuals had limited contact with health centres, it would be
important to move interventions beyond clinic settings and into the
community.

I haven't seen the game yet, but I actually felt that way (breathing easy), the 2nd half
of the Pats game. It was weird, knowing the Pats offense & Jets history, but I never

H, when one of my few complaints was Slausson running into LT when he
was supposed to have pulled & delivered a block, well, you know it was
easy. He did cost us a TD on that play.

btw Slausson appeared to do OK today.

It is amazing how smart an OC looks when players catch the ball, QB's
make the right decision, etc.

It shoudl have been 20-0 at halftime. You always take the points.
Maybe you throw it with 3rd & 6 but LT averaged 7 yards a carry
yesterday. Maybe you don't pass it 3 & goal from the 1 but you take
the points.

MZ, if we are talking about the same series I didn't like the throw in
the end zone (2Q) but if you saw on one down Slausson knocked LT over
because Slauson can't move. So if they score a TD because the players
execute then it is a good drive?

As for running on 3rd & long, well, I think the team philosophy is not
to make a mistake. If you run on 3rd & long doesn't that occasionally
open up when you do pass? I don't want to see it every time but it
has its place especially with LT.

I wasn't a big fan of using the wildcat down at the gola line.
Yeah... I know it worked, but we would be screaming our heads off and

look, I'm not defending the move and still would like to know the
*real* reason they cut Faneca loose, but the Cardinals gave up 9 sacks
yesterday and rushed for less than 50 yards against the Chargers.
Slauson gave up no sacks for the second straight game and committed no
penalties. This is his fourth career start. I still think we vastly
overrate Faneca. But I admit, Tanny saved no money by cutting him and
the team is pretty thin on the oline.

I will shut up about Washington. He picked the right older back so
maybe we should trust him on McKnight. btw he looks to have added
some size. McKnight not Rex.

did much better in NY.

i saw mcnight play in a few games. i thought he looked promising.
what i was wondering is why carol did nothing to bring him along to
seattle. my feeling was this... since mcnight displayed some pro
quality talent, perhaps there was a work ethic or attitude issue that
carol saw as a coach that rex/tanny could not see from further away.
and... may be :-) rex is a better coach than carol is with guys like
that and he will coach mcnight up in spite of himself.

He is very similar to Leon in many ways. One of the biggest HS
recruits in the country that did OK in college. Their numbers are
almost identical with McKnight's slightly better. McKn can run as he
showed in some of his returns in pre-season. Will he work hard
enough?

As for Carroll, my guess is you hit the nail on the head. Let's hope
Rex can play Emma Lazarus & take some of these strays in & make it
work.

i called mcnight "a serious piece of manpower" when he was drafted
based on what i saw him do at usc. i hope rex and co can get him to
produce or you can add mcnight to the list of guys i was wrong
about... current guy is bradford. i thought it would take him a while
to get up to speed and clausen would out-do him early on :-)

i'm still sticking with slauson, though... he had a nice game
yesterday and helped open up a few king sized holes.

I don't think we need a third RB really, I think we need what you
said, a bigger work load for greene. IMO 3 back tandems just prevent 2
of the guys from having more of a feel for the game. We also run Brad
Smith from time to time out of the wildcat (btw, I hate that name,
it's an option play) I looked it up and LTII as in on twice as many
snaps as Greene. I know it's hard to not go to LTII when he gets off
to these fast starts but maybe some patience with Greene is all you
need to have. the guy does rip some big runs.

i dont expect gholston will be on the roster next season. he may not
even be on any nfl roster. with rex being well known at developing
defensive guys... even guys that dont produce at first... what is left
for a player that cant get it done after rex has had his try ??? may
be he'll get a low cost deal from a team that is very thin at olb-de

Not feeling the love in New York: wide receivers Patrick Turner and David Clowney, who
were released by the team on Monday to help make room for Holmes. The Jets will also
re-sign defensive end Howard Green.

The Jets finished their early round robin in the AFC East with three wins, including two
on the road. They've taken early control of the division despite all the missed games

I don't think so. They were planning to sign him today, even before Ellis' injury.
We should know about him today, but all the early reports were that it's a mild knee
sprain. They need some bods on the DL, so I suspect Green will be comin' & goin' a lot
this season, depending upon the opponent, & our specific needs for that opponent.
The Revis injury still concerns me. I'd give him, at least, one more game off, especially
with the Vikings next. I haven't seen them, but they don't seem to be very strong at WR.
They'll need Revis more against the Broncs... then it's the Bye.

I don't think they'll need him against the Broncos either. They should
let him sit right through the bye, and then bring him back so he's ready
for the divisional games.

I really don't pay much attention outside the division anymore. Aren't the Broncs airing
it out? And isn't Orton playing well?
As an aside, it's looking like the Broncs made out better than expected, when they traded
Cutler.

Sex Survey Finds Surprising Changes Between the Sheets in America

"When Harry Met Sally" is a movie classic, and its infamous faked
orgasm scene still resonates with couples.

Now, an Indiana University survey published today has put some
confirming data on the faked orgasm phenomenon. According to the
survey's researchers, 85 percent of men said their partners climaxed
during the most recent sex act, while 64 percent of women reported
they actually did.

"There's this massive gap between men's perception and women's
reality," says Debby Herbenick, co-author of the research and
associate director at the Center for Sexual Health Promotion at the
university. "It shows a lack of communication between partners, either
by women faking it, or by men not asking or noticing if their partner
[climaxed]."

The survey, which drew on data from nearly 6,000 participants between
the ages of 14 and 94, covered a wide range of sexual behaviors,
sexual health practices and sexual perceptions, and according to the
center, it was the largest nationally representative survey on sexual
health ever performed.

It's worth noting, however, that the survey was sponsored by the
makers of Trojan condoms, which had some influence over the design of
questions, particularly those covering condom use.

to capture contemporary sexual behavior. One of the frustrating things
in sexual research up to this point is that the best data is from the
early 1990s, and a lot has obviously changed since then," says
Herbenick.

The new research offers a glimpse of what's going on between the
sheets for Americans today and provides not only a wide range of
information on sexual behavior but also how these behaviors shift over
time.

As for the perceptual gap regarding female orgasm, this finding
confirms past research on the subject, as well as cultural
perceptions, says sex expert Pepper Schwartz.

"I think its a combination of people being too embarrassed to work out
an issue or too careful about shaking up the system by giving accurate
feedback. Men take a lot of pride in 'giving' a woman an orgasm ... so
in the beginning, faking it could be about encouragement."


The Wide World of Getting It On
Teens are doing it, octogenarians are doing it, and if this national
survey shows anything, it's that everyone's getting it on in one way
or another. Some of the most striking findings among the Indiana
University team had to do with the prevalence of certain sexual
behaviors that had not been as frequently revealed in previous data.
For instance, in the new survey, it was found that men performed oral
sex almost as frequently as they received it, says Michael Reece, co-
author of the study and faculty member at the Kinsey Institute.

"It hasn't really been part of our cultural script for a while, this
idea that men are giving oral sex close to the amount that they
receive it, but it looks like that's really changing," he says.
